Provider Resources
Destinations & Vouchers
Dear Providers,
You are the most valuable member of our team! You are on the frontlines, ensuring that every member in the program is receiving the right support to help them thrive and support the family!
Every session is a chance to make a difference in a child’s life. HCBS stands for Home & Community Based Services as the goal is to help the member thrive at home and in the community.
To that end, we have compiled resources, vouchers, and outing suggestions to help you connect to your member and engage with them while working towards the goals of the specific service type you're providing!
Please see below.
Don’t see options in your area or have a local outing you want to request funding for? Email us at staff@fssny.org to make requests!
